Navigating Your 2027 Medicare Benefits: What’s New and What It Means for You

Three people reviewing insurance documents at a desk.

Every year, Medicare introduces updates to its premiums, deductibles, and plan structures. The 2027 calendar year brings several major adjustments, particularly concerning prescription drug costs and plan availability.

Understanding these updates before or during the Annual Enrollment Period is the best way to protect your health and your budget. Here is a straightforward breakdown of the most significant changes you can expect starting January 1, 2027.

1. Prescription Drug Changes (Part D)

Prescription medication updates are taking center stage this year due to ongoing rollouts under federal drug pricing guidelines.

  • The Out-of-Pocket Cap Rises slightly: In 2026, the annual out-of-pocket maximum for covered medications was capped at $2,100. For 2027, the Centers for Medicare & Medicaid Services (CMS) has increased this maximum cap to $2,400. Once you reach this limit, you pay $0 for your covered medications for the rest of the year.

  • Deductibles are Adjusting: The maximum allowable deductible for a standard Part D plan is rising to $700 (up from $615).

  • Negotiated Drug Prices Take Effect: Medicare has negotiated lower, direct pricing for 15 additional high-cost brand-name drugs, including popular medications like Ozempic.

  • Insulin Safeguards Remain: The predictable $35 monthly copay cap for covered insulin remains firmly in place. [1]

2. Changes to Doctor and Outpatient Costs (Part B)

For individuals enrolled in Original Medicare, out-of-pocket costs for regular doctor visits and outpatient care are experiencing modest baseline increases. [1]

  • Monthly Premiums: The standard Medicare Part B monthly premium is projected to increase to approximately $209.50 (up from $202.90).

  • Annual Deductible: The annual Part B deductible is projected to rise slightly to $292 (up from $283). [1]

3. Medicare Advantage Plan Disruption (Part C)

While CMS projects that national average Medicare Advantage premiums will remain relatively low (dropping to an enrollment-weighted average of $12), the internal structure of local plans may look very different. [1, 2]

  • Fewer Plan Options: Major private insurance carriers are scaling back their service regions or exiting certain markets entirely. This means some popular local plans are being discontinued. [1, 2]

  • Shifting Networks and Copays: Even if your plan continues into 2027, insurers are adjusting their networks of doctors, increasing copays, or reducing extra "supplemental" perks like dental or vision allowances to manage costs. [1, 2]

4. Telehealth Access Extended

As a piece of welcome news, expanded access to Medicare telehealth services has been extended through December 31, 2027. You can continue to consult with many of your preferred healthcare providers virtually from the comfort of home. [1]

Your Next Steps: Avoid the "Autopilot" Mistake

Because so many local plans are changing their premium costs, pharmacy networks, and drug coverages, renewing your current plan automatically without checking it could lead to unexpected costs. [1, 2]

  1. Read Your ANOC: Look for your Annual Notice of Change (ANOC) packet in the mail or your email inbox. It explicitly lists exactly what your current plan will cost and cover next year.

  2. Review Your Medications: Make a list of your current daily medications and check them by calling the Douglas Insurance Group to ensure they are still covered on your plan's formulary.

  3. Take Action Before the Deadline: The Medicare Open Enrollment window runs from October 15 through December 7. Any plan selections or adjustments you make during this window will officially go into effect on January 1, 2027.
